AWARD CATEGORIES
LES REYNOLDS COMMITMENT TO EXCELLENCE AWARD
This award was established to honor Les Reynolds, founder of the Safety Council of the Ozarks, who was committed to Making Our World Safer. The Les Reynolds Commitment to Excellence Award was established to recognize those who worked to make the world safer and is the highest regional award presented by the Safety Council of the Ozarks.
The criteria is based on the Experience Modifier, the Total Case Injury and Illness Rate (TCIIR) and the Days Away Restriction Transfer (DART). An average of the most recent three calendar years will be compared to the average industry rate. The company with the largest percent difference between their industry average and their internal record will be recognized with this award.
A winner will be selected from each category listed below:

| Category II | 50,001 - 100,000 hours |
| Category III | 100,001 - 350,000 hours |
| Category IV | 350,001 + hours |
LES REYNOLDS MILLION HOUR AWARD
Companies achieving one million or more hours without a loss work incident will be recognized with this award.
OZARKS REGIONAL SAFETY EXCELLENCE AWARD
The Ozarks Regional Safety Excellence Award will be presented to those companies that strive to provide a culture of safety at their workplace. The criteria for this award is based on the Total Case Injury and Illness Rate (TCIIR) and Days Away Restriction Transfer (DART). The average of the most recent three calendar years will be compared to the average industry rate. The top four companies with the largest percent difference between their industry average and their internal record will receive this award.
SAFETY CHAMPION AWARD
Do you have a Safety Champion in your company? Someone who always practices safety, puts their training to use and motivates others to be safe? In one page, tell us why you feel they should be recognized and submit your entry to the Safety Council by the award deadline.
SAFETY TEAM AWARD
Do you have a safety team that is doing an outstanding job? In one page, tell us why your safety team/committee should be recognized and submit it to the Safety Council by the award deadline. The judges will review all applications and select a team to be recognized at the ceremony

APPLICATION INFORMATION
Eligibility:
Applicants must be a member of the National Safety Council and its Chapter Network OR located in the Safety Council of the Ozarks service area. To confirm that you are a member or located in the service area, contact the Safety Council at 1-800-334-1349 or info@nscozarks.org.
Divisions of corporations may submit a separate application for each facility if they can certify that:
- They operate in a separate location.
- Have a distinct line of business identity for the division.
- Are managed independently from other divisions of their corporate parent for safety and loss control (i.e. independent procedures and programs for safety).
AWARDS PRESENTATION:
Award winners will be announced during an awards lunch at the Mid America Safety, Health and Environmental Conference and Expo. All award winners must agree to attend the Awards Lunch on Thursday, May 3, 2012, noon to 2:00 p.m., Tan-Tar-A Resort, Lake Osage, Missouri.
JUDGES:
A panel of safety and health professionals will judge application papers.

APPLICATION DEADLINE:
All applications must be received by the Safety Council of the Ozarks no later than March 30, 2012. Late entries will not be accepted.
